If her language skills are impaired then she does not have aspergers. She may have PDD-NOS or may have autism.
Are her parents going to get her tested? You could try encouraging them to do so, but many will get very defensive so be prepared for that.
Why does it matter? Either way she will have difficulty dealing with this chaotic world. This is a spectrum disorder. Think of it as a rainbow and she falls somewhere along all of the colors. There is not a blood test, so the observable criteria are merely a way of diagnosing. So again, it does not matter which it is. It is like saying "dried plums" rather than "prunes."
The parent who posted just before me had some good info - kids do change a lot. Who she is now and what she displays may be completely different in 5 to 10 years time. Regardless she needs intervention now - the sooner the better.Is this Asperger's or PDD-NOS?

She sounds like she is autistic, not Asperger's. You may want to go online and see if you can find a rating scale like the CARS, GARS or GADs.Is this Asperger's or PDD-NOS?
as a mother of a child with aspergers we have a saying about children on the spectrem No two children are alike. Our children do Not have to Have every single characteristic on the checklist to have a diagnosis.Thats for thr profesionals to figure out . My son is 16 and has a DX of aspergers and what he may have exhibited at age 10 He may not exhibit Now .For instance He ahas Many Facial Featurs he has empathy for others .These checklist are just something to go By ther not set in stone . My son went from PDD~NOS to aspergers In a Matter Of a 5 year span .All im saying is dont let some checklist stear you to or from getting the correct service for your loved ones.
There's an overlay of the exbitions she is showing. At the age of four there could be alot of reasons why she is presenting these characteristcs you listed. Is your cousin in Pre-k? Once in school is easier to know for sure how she interacts with others as well as her cognitive and motor skills. It owuld be helpful to start some of these tests and analysis to be aware. Depending on the state you live in, some school districts do those tests for free. It all depends in various factors. Moreover, once the parents are aware your cousin can et the help she needs, if she needs help at all. It is important to remember that at the age of four some characteristics are normal, and some are not. It is why it would be ideal to start the testings as soon as possible. If in fact your cousin has somehting such as Asperbergs, it is important to do as much reading and a lot of comprehension will be needed to fully understand what goes on as well as what needs to be done at home. No one can "fix" your cousin, and hopefully your family understands that and are patient with the four year old. Wishing you the best.
